Camp Forms

Health History (including immunizations) is to be entered digitally and can be completed in your camp account by the parent/guardian.

Physician Examination Form

Medication Form (required if any medications will be brought to camp)

Immunizations may also be provided by uploading a pdf of of the school record in your online account or they can be emailed or mailed.

Complete and return all forms by June 15.

-Sheet set (Twin or Cot sized)
-Sleeping Bag (1)
-Extra Blanket

-Laundry Bag (1)
-Backpack
-Flashlight (with extra batteries!)
-Bug Spray
-Sunscreen
-Water Bottle
-Pen, paper & pre-addressed, stamped envelopes for writing letters home. (suggested)
-White t-shirt for tie-dye
